By registering for programs at Chiqui Social, I acknowledge that I have read and fully agree to all registration terms and conditions stated herein.
Non-compliance with Chiqui Social terms and conditions may result in termination of registration. Chiqui Social has the right to update the terms and conditions at any time. Classes and staff are subject to change without notice.
FEES & REGISTRATION
Registration: Please follow the registration process online. You will be invoiced upon successful enrollment in the program.
CANCELLATION POLICY
Thank you for your consideration for other children on the waiting list and the preparation by our teachers as you consider your attendance at camp. As you sign your child up for camp, please know that we are looking forward to a great camp and want to be sure each child is ready and able to attend in advance of your signing up. We understand if you have to cancel and we will process your refund minus an administrative fee prior to our cut-off timeframe (see below).
Full Week Camp: If a cancellation occurs 4 full weeks (30 days) or more prior to your scheduled camp session, a refund will be provided minus a $75 administrative fee. Refunds will NOT be provided after this cut-off time. Unfortunately, we cannot accommodate refunds for partial attendance, sickness. Special exceptions may be granted depending on the circumstances. Chiqui Social® reserves the right to cancel any program due to low enrollment. Upon our cancellation, a full refund will be issued.
PERSONAL BELONGINGS
PLEASE LABEL your bag, all shoes, and belongings brought to the studio or camp, Chiqui Social is not responsible for any lost or stolen items.
MARKETING RELEASE
I understand that images of the participant may be used in Chiqui Social marketing materials, promotional videos, social media or website materials, These images will be used for Chiqui Social purposes only, and will not be given or sold to outside companies or individuals. I agree that no form of payment will be owed to me for use of any images in which my child is included. Please send the studio an email if you prefer to opt-out.
*NO CHILD’s NAME WILL EVER BE ASSOCIATED WITH THE IMAGE/VIDEO USED UNLESS A SEPARATE RELEASE IS ATTAINED.
ASSUMPTION OF RISK
I acknowledge and fully understand that I, or my child, will be engaging in physical activities that may involve some risk of injury. I acknowledge and have been advised that it is my responsibility to consult with my child’s physician with respect to any past or present injury, illness, health problem or any other condition or medication that may affect my or my child’s participation. I assume the foregoing risks and accept full personal responsibility for any personal injuries sustained by my child or myself which might incur as a result of participating in the Chiqui Social program.
RELEASE OF LIABILITY
I understand the risks and benefits of physical activity and state that all family members participating are in a condition to play. In consideration of the named member being permitted to participate in Chiqui Social activities, we, the parents or legal guardians of the member, give my approval for the family member’s participation in any and all activities of the program. On our own behalf and on behalf of the student hereby acknowledge and agree that we understand and fully appreciate the risk of injury involved to the student participating in Chiqui Social activities.
It is the express intention of the parent or guardian to forever waive, and forever release and discharge and hold harmless Chiqui Social, its owners, directors, members, employees, professional consultants, and independent contractors from any claim, cause of action or liability for any and all loss, liability, damages, injuries, or costs and expenses including attorney’s fees they may incur. The undersigned acknowledges that he or she is fully aware of the legal consequences of signing this express waiver and release of liability.
MEDICAL CARE WAIVER
I agree to provide health and safety issues regarding camper allergies, physical limitations, or medication required: I understand the risks and benefits of physical activity and state that all campers participating are in condition to play. In consideration of the named member being permitted to participate in Chiqui Social activities, we, the parents or legal guardians of the member, give my approval for the camper’s participation in any and all activities of the program.
MEDICAL INSURANCE
Chiqui Social LLC, does not provide any medical insurance to cover medical expenses incurred by the camper. The camper’s family medical insurance policy must cover any such costs that may be incurred.
MEDICAL AUTHORIZATION
When leaving a child in our care, the child’s parent/legal guardian signing this form consents to Chiqui Social, or any other employee, agent, assistant, or other person associated or affiliated with Chiqui Social to seek medical attention and/or treatment or other measures deemed necessary or advisable in the discretion or judgment of Chiqui Social, LLC for named camper, in the event of an accident, sudden illness, or any other condition that occurs while the camper is in the care and supervision of Chiqui Social, LLC personnel. The parent/legal guardian further understands that Chiqui Social will make reasonable efforts to notify the parent/legal guardian or another parent of the camper in the event of any incident that may require health care or treatment of the camper; however, such parent/legal guardian understands that such notification may not be able to be provided under the circumstances prior to such health care and/or treatment. The parent/legal guardian signing this form releases Chiqui Social LLC, and all of its owners, agents, personnel or other persons affiliated therewith, from any and all liability for such health care decisions or actions in seeking medical care and treatment for the camper, and specifically agrees to pay for all costs and fees that may be charged or incurred for such medical care and treatment authorized under this Medical Authorization.
LIABILITY WAIVER AND RELEASE
The parent/legal guardian hereby agrees to release and hold Chiqui Social LLC, its owners, agents, assistants, helpers, personnel, and other persons affiliated therewith, including, but not limited to, from any and all claims, damages, losses, and/or expenses, arising out of the camper’s participation in camp activities, and assume any and all liability for any and all personal injuries, bodily injury, illness or property damage that occurs as a result of participation in any camp activity. The parent/legal guardian also warrants and represents that participation in camp activity by the camper is voluntary and the camper and his or her parent(s) or legal guardian understands that there may be some risk involved in some camp activities. Each camper agrees to obey all the rules and policies mandated by camp personnel. The parent/legal guardian understand and agree that all rights under Section 1542 of the California Civil Code of Procedure, are hereby expressly waived. Said Section reads as follows: “Certain claims not affected by the general release. A general release does not extend to claims which the creditor does not know or suspect to exist in his favor at the time of executing this Release, which if known by him must have materially affected his settlement with the debtor.”
I have carefully read this document and fully understand its contents. I am aware that this is a Release of Liability and an Authorization of Emergency Medical Care and Treatment of my child or ward. I have signed this document voluntarily and of my own free will. I am not relying on any representations, statements or inducements by any person associated with Chiqui Social. I do certify that I am the parent or legal guardian of the camper described above and I agree to all the terms of this document.
SICK POLICY
These symptoms require absence:
Fever: Children will be sent home if their temperature is 100.0 or higher. Children must be free of fever (any temperature above 98.6 degrees) for at least 24 hours without the use of fever-reducing medication. The same policy applies if your child develops a fever at home. They must be fever free (any temperature above 98.6 degrees) for at least 24 hours without the use of fever-reducing medication.
Rash: Any rash other than a common diaper rash or skin irritation will require that child to be sent home. They may return based on that written doctor’s evaluation and clearance that it is not contagious.
Conjunctivitis (pink eye): Children will be sent home if there appears to be an unusual amount of discharge from or irritation to their eye(s) and must stay home the next day for observation. Before returning to they will need an evaluation and diagnosis from their doctor in writing of exactly what it is.
If the diagnosis is BACTERIAL CONJUNCTIVITIS children must have received at least 24 hours of treatment. If the diagnosis is VIRAL CONJUNCTIVITIS your child may return AS LONG AS THERE IS NO DISCHARGE. If in fact, they do not have “pink eye” we need a doctor’s note with a diagnosis and a clearance that it is not contagious.
Thick White, Green or Yellow Discharge: Children will be sent home if they appear to have any thick white, green or yellow discharge. This is often indicative of an infection and they must stay home the next day for observation. Before returning they will need an evaluation and diagnosis from their doctor in writing and at least 24 hours of treatment. If in fact, they do not have an infection we need a doctor’s note with a diagnosis of exactly what it is with a clearance that it is nothing contagious. The Department of Health and most doctors are of the opinion that once on antibiotics for 24 hours, the discharge is no longer contagious even though it may persist for up to two weeks.
Diarrhea: Children will be sent home if they have loose bowel movements. Before returning children must be free from diarrhea for 24 hours with at least 1 regular bowel movement. If your child has one or more loose bowel movements on their first day back they will again be sent home.
Vomiting: Children will be sent home if they vomit and must stay home the next day for observation. Before returning (after the day of observation) children must symptom-free with no vomiting for at least 24 hours.
Persistent Hacking Cough: Children will be sent home if they have a persistent hacking cough. Before returning they will need an evaluation and diagnosis from their doctor in writing and at least 24 hours of treatment. If in fact they do not require any treatment we need a doctor’s note with a diagnosis of exactly what it is with a clearance that it is not contagious.
Lice: Children will not be readmitted until 24 hours after treatment and must be nit free. The Director will make an evaluation and determine if the child can be readmitted.
